Volunteer Applications for New and Returning Applicants are

Open for the 2024-2025 School Year

 

If you would like to volunteer on campus with students and during school hours at Fairburn Ave Elementary School, you must complete the LAUSD volunteer application process. Follow the steps below to get approved.

 

New Volunteers

  1. Create a Parent Portal account if you do not have one already.
    • NOTE: Families new to LAUSD will be able to create a parent portal account in August before their child begins school.
  2. Complete online volunteer application(Print or Save & Sign - save for step 6)
  3. Complete Volunteer Commitment Form(Print OR Save & Sign - save for step 6)
  4. Complete the Liability Waiver(Print OR Save & Sign - save for step 6)
  5. Take a TB test (within 60 days prior to your application) and complete this TB form.
    • Tests can be taken with your regular healthcare provider or at places like Wellness Mart. 
  6. Submit ALL documents to Ms. Shawntee in the main office (skm2717@lausd.net)
    • Online application from step 2, signed (e-signatures accepted)
    • Volunteer Commitment Form from step 3, signed (e-signatures accepted)
    • Liability Waiver from step 4, signed (e-signatures accepted)
    • TB Test Form

Returning Volunteers

If you were an approved volunteer at Fairburn Ave previously and your TB test is still valid (within the last 4 years), please complete the following steps:

  1. Complete the online volunteer application(Print or Save & Sign - save for step 4)
  2. Complete Volunteer Commitment Form(Print OR Save & Sign - save for step 4)
  3. Complete the Liability Waiver(Print OR Save & Sign - save for step 4)
  4. Submit ALL these documents to Ms. Shawntee in the main office (skm2717@lausd.net).
    • Online application from step 1, signed (e-signatures accepted)
    • Volunteer Commitment Form from step 2, signed (e-signatures accepted)
    • Liability Waiver from step 3, signed (e-signatures accepted)
